2010 GOLDEN MIKE AWARDS

1. Cover letter with the station’s or individual's complete list of entries.

2. Check for payment of entry fees in the correct amount made payable to "RTNA" if payment has not already been made on-line.
   NOTE: ENTRIES RECEIVED WITHOUT ENTRY FEES WILL BE DISQUALIFIED!

3. Entry Form for each entry filled out on-line, printed, and signed by the News Director for station entries and the entrant for individual entries including the name, title, and phone number of the News Supervisor who can verify the entry.
   3 copies of the newscast rundown for Best Newscast entries (TV-1, TV-2, TV-3, R-1, R-2).

4. Entry DVD or CD for each entry that heeds the technical requirements in Rule 7, sections 7.7 and 7.8, and that has a label affixed with the following information:
   • Category No.
   • Division (A or B)
   • Date of Broadcast
   • Entry Title
   • Length
   • Station Call Letters (including AM, FM, TV)
   • Contact Name
   • Telephone
   • E-mail

   Note: All of the above information is on the top half of your completed Entry Form, which may be photocopied and reduced to fit on entry DVDs and CDs.

5. Rule 1.5 Form printed off the website and signed by the News Director for Station entries and the entrant for individual entries.
